> A Senate request raises questions about military intelligence. The White House's June policy sets current testing requirements; its implementation is a separate oversight question.

Senators Jack Reed, Mark Warner and Chris Coons want inspectors general to investigate reported AI failures in U.S. intelligence and military operations. Their [September 21 release](https://www.reed.senate.gov/news/releases/reed-warner-and-coons-seek-investigation-into-faulty-ai-assisted-intelligence_operations) calls for unrestricted access to reported and potentially unreported cases.

## The big change

- **What changed:** The proposed scrutiny would follow AI into the intelligence commanders receive, before any decision to use force.
- **Why it matters:** Human command does not settle whether the information behind a decision is sound. That distinction matters for soldiers and civilians.
- **What to watch:** Evidence of how agencies apply the administration's testing requirements, alongside any investigative findings. A written reliability rule cannot show how a particular system performed.

## What prompted the request

The senators cite reports of outdated geospatial data in the Minab strike and possible AI-generated falsehoods in intelligence preceding an aborted military interdiction. Their letter seeks root-cause analysis; it does not announce an inspector-general investigation or establish AI causation.

The Minab case concerns the February 28 attack on an Iranian primary school. [AP's July 1 reconstruction](https://apnews.com/article/2a134a5c74d80db763db4c3eb6d0d847) reported that at least one U.S. missile hit the school, killing more than 100 children. AP drew on video, open-source evidence and interviews. AP's reconstruction does not establish which AI system, if any, contributed to the attack.

## The White House still requires testing

President Trump's [June 5 national-security AI memorandum](https://www.whitehouse.gov/presidential-actions/2026/06/national-security-presidential-memorandum-nspm-11/) replaces the previous National Security Memorandum 25 and directs faster adoption. It also requires testing, evaluation, validation and verification. Section 2(c) explicitly demands reliable, controllable systems.

The same document sets 90-day deadlines for updated autonomous-weapons guidance and a policy governing AI in national-security systems. The memorandum itself does not show whether agencies completed those steps.

Section 2(d) separately holds commanders and agency heads accountable for its privacy and civil-liberties obligations.

The June directive postdates the school strike. It supplies current requirements; investigators must establish which rules applied in February.
